valid e-mail address. If you do have a site, you need to
put up a code, either text or image, prior to or right after submitting. If I check your site and the code is not there, I will only list your e-mail address.
3. If you have a site, I will
not tolerate pornography, homophobia or any form of hate.
4. When filling out the form,
do not use shortforms like "D," long aliases or initials. Just a simple first name is fine, or a short alias is acceptable.
Please do not include your last name.
5. If you ever change your e-mail address or URL, you
must fill out an
Edit Form.
